Prior to admission the following  forms must be completed.

• Copy of Birth Certificate
• AKD Registration Form
• Copy ofImmunization Record
• Student’s proof of Residency 
• Emergency Contact Form
• Medication Release Form
• Copy of Parent’s (Guardian’s) Birth Certificate
• 6 Portrait Photos 
• 6 Parent Authorization of Administration of Medication at School
